Transitions services are:
- Designed to improve or maintain the participants optimal level of functioning in socialization, adaptive, and self-help skills
- Provided in small group settings (1 to 4 staff to participant ratio) with caring, experienced staff
- A result of collaboration and cooperation among the participant, their families, and service providers

About Us
Transitions is an adult care facility that offers planned activities, supports, training, and supervision to adults with intellectual disabilities.
We accept individuals who range in age from eighteen to fifty and can benefit from a structured environment. These individuals have a primary diagnosis of mental retardation/intellectual disability, developmental disabilities, with related conditions. We can accommodate individuals who are ambulatory and non-ambulatory and exhibit mild to moderate medical conditions.
Transitions is an approved and enrolled provider of day support services under the Mental Retardation and Developmental Disabilities Waivered Service Program.Program operations are licensed through the Virginia Department of Behavioral Health and Developmental Services www.dbhds.virginia.gov
